3. Thai Buddhist monk Identification Card
4. Letter from the National Office of Buddhism
5. Resolution from the Sangha Supreme Council (for Thai Buddhist monk missionary only)

7. Applicants may collect the passport in person or request for the return of passport by post (For postage returns, please provide a self-addressed pre-paid Royal Mail Special Delivery 500g envelope)

Note:    

- Thai Buddhist monk missionary may apply for both an official passport and an ordinary passport

- Other monks may only apply for an ordinary passport
